ต่อมไทรอยด์คืออะไร?

ต่อมไทรอยด์ (Thyroid gland) เป็นต่อมไร้ท่อขนาดเล็ก รูปร่างคล้ายผีเสื้อ อยู่ที่บริเวณลำคอด้านหน้า
หน้าที่หลักคือการผลิต “ฮอร์โมนไทรอยด์” ซึ่งเป็นตัวควบคุม ระบบเผาผลาญ และ การทำงานของอวัยวะทั่วร่างกาย

ฮอร์โมนไทรอยด์แต่ละตัวคืออะไร?

ต่อมไทรอยด์สร้างฮอร์โมนหลัก 2 ชนิด: T3 และ T4 โดยการทำงานจะสัมพันธ์กับฮอร์โมนจากต่อมใต้สมองชื่อ TSH

1. TSH (Thyroid Stimulating Hormone)

  • สร้างจากต่อมใต้สมอง ทำหน้าที่ “กระตุ้น” ต่อมไทรอยด์ให้ผลิต T3 และ T4
  • หากไทรอยด์ทำงานน้อย → ร่างกายจะหลั่ง TSH มากขึ้นเพื่อกระตุ้น
  • หากไทรอยด์ทำงานมาก → TSH จะลดลง

2. T4 (Thyroxine)

  • เป็นฮอร์โมนหลักที่ผลิตจากต่อมไทรอยด์ (มากถึง 90%)
  • อยู่ในรูปที่ยังไม่ออกฤทธิ์ ต้องแปลงเป็น T3 ก่อนจึงจะใช้งานได้

3. T3 (Triiodothyronine)

  • เป็นฮอร์โมนที่ออกฤทธิ์จริง ควบคุมการเผาผลาญ พลังงาน อารมณ์ และสมอง
  • แม้มีปริมาณน้อยกว่า T4 แต่มีความแรงในการทำงานมากกว่า

4. Free T3 / Free T4

  • คือฮอร์โมนในรูป “อิสระ” ที่ร่างกายนำไปใช้ได้ทันที
  • เป็นค่าที่ใช้วิเคราะห์การทำงานไทรอยด์ได้แม่นยำกว่าฮอร์โมนรวม

5. Anti-TPO / Anti-Tg

  • เป็นแอนติบอดีที่บ่งบอกว่าอาจมี “โรคไทรอยด์จากภูมิคุ้มกัน” เช่น
    • โรค Hashimoto’s thyroiditis (ไทรอยด์ต่ำ)
    • โรค Graves’ disease (ไทรอยด์เกิน)

ภาวะที่เกี่ยวข้องกับฮอร์โมนไทรอยด์

Hypothyroidism (ไทรอยด์ต่ำ)

ร่างกายผลิตฮอร์โมนไทรอยด์ไม่พอ → เผาผลาญช้าลง
อาการ:

  • เหนื่อยง่าย ขี้หนาว น้ำหนักขึ้น
  • ผิวแห้ง ผมร่วง รอบเดือนผิดปกติ
  • ซึมเศร้า สมองช้า ขี้ลืม

สาเหตุที่พบบ่อย:

  • Hashimoto’s (ภูมิคุ้มกันทำลายต่อมไทรอยด์)
  • ขาดไอโอดีน
  • เคยได้รับการฉายแสง หรือผ่าตัดไทรอยด์

Hyperthyroidism (ไทรอยด์เกิน)

ร่างกายผลิตฮอร์โมนไทรอยด์มากเกิน → ระบบเผาผลาญเร็วผิดปกติ

อาการ:

  • ใจสั่น มือสั่น เหงื่อออกง่าย
  • น้ำหนักลดเร็ว กินเยอะแต่ผอม
  • หงุดหงิด นอนไม่หลับ
  • ตาโปน รอบเดือนน้อย

สาเหตุที่พบบ่อย:

  • Graves’ disease (ภูมิคุ้มกันกระตุ้นต่อมไทรอยด์มากผิดปกติ)
  • เนื้องอก หรือก้อนในไทรอยด์บางชนิด

การตรวจไทรอยด์ (Thyroid Function Test) คืออะไร?

  • เป็นการเจาะเลือดเพื่อตรวจฮอร์โมน TSH, Free T3, Free T4
  • อาจตรวจ Anti-TPO / Anti-Tg หากสงสัยภาวะไทรอยด์จากภูมิคุ้มกัน

ตรวจเพื่ออะไร?
